To assist you better when contacting our office, please provide the patient’s name and the account or call number. Due to HIPAA regulations, we are only able to communicate directly with the patient unless permission to speak to someone else is received in writing.
If you are unable to pay your bill, you may apply for a reduction in charges based on your income. Please print the Application for Reduction in Charges form, fill out and mail to our billing office.
