SUBSCRIPTION PROGRAM

Overview

Ambulance service is something most people don’t think about, until an emergency occurs. And during an emergency, no one wants to think about whether our services are covered regardless of whether you have insurance or not. In order to help, we offer an affordable annual subscription program which we encourage you to review and consider. As a subscriber, you will enjoy piece of mind, and at the same time you will be helping to support Upper Valley Ambulance’s mission in the communities we serve.

Subscription programs were created to offer protection to members by controlling out of pocket expenses. For an annual fee, subscribers can join and be relieved of expenses such as insurance co-payments and deductibles for medically necessary emergency ambulance transports.

This program is not an insurance policy. The difference between a subscription program and an insurance policy is that with a subscription, UVA files the subscribers insurance claims for ambulance services and will accept insurance as payment in full. UVA will not bill subscribers for co-payments, deductibles or for partial payments made by insurance companies.

Protection from denials is a major benefit of this program. It is not uncommon for insurance companies or Medicare to deny between 20 and 40 percent of ambulance claims submitted. This may mean that the entire ambulance service bill becomes the patients responsibility. UVA subscribers are protected from denials of medically necessary emergency transports.

Subscribers can rest assured that is their medically necessary emergency transport is not fully paid or even denied, they will not be responsible for payment.

For your subscription, Upper Valley Ambulance, Inc. agrees to provide all persons living in your household with unlimited medically necessary Emergency Ambulance Service for one full year. Subscriptions are accepted anytime through the year, and expire in either June or December of the next year.

Who is eligible ?

Only those residing in the communities of Bradford, Corinth, Fairlee, Thetford, Vershire, West Fairlee Vermont, and Orford and Piermont, NH are eligible for the subscription program.

Who is covered?

The principle subscriber and all persons residing in your household will be covered under this subscription.

Cost of Service:

Once a year each household buys one $45.00 Subscription to Upper Valley Ambulance, Inc.

What about Insurance Coverage?

Health Insurance may cover a portion of the ambulance bill, but generally only under Major Medical. With most Insurance Companies there is a deductible amount.

What If I do Not Have Insurance?

COST TO SUBSCRIBERS NOT COVERED BY INSURANCE IS $45.00 PER HOUSEHOLD.

Upper Valley Ambulance reserves the right to any third part billing. Upper Valley Ambulance, Inc. agrees to bill your insurance company for you as part of your Subscription Service Agreement. (The subscription agreement requires you to send Upper Valley Ambulance, Inc. any insurance payments received by you for service performed by Upper Valley Ambulance, Inc.)

Insurance payments account for a large portion of Upper Valley Ambulance’s operating funds.

A Subscription Agreement provides unlimited medically necessary Emergency ambulance service for all persons residing in your household, and is renewable in June or December of each year.

All checks should be made payable to Upper Valley Ambulance, Inc.

A member is Never Charged for the balance of a medically necessary ambulance bill not covered by insurance.

• Upper Valley Ambulance, Inc. serves 9 communities in New Hampshire and Vermont.

• Free Home Safety Inspections, Blood Pressure checks, CPR instruction and tours of our headquarters are available upon request.
